YOUR FIRST APPOINTMENT

At your first appointment, you will be asked to provide a photo ID, insurance card (including supplement, if applicable), and physician's order for therapy services; then, there will be a few forms for you to complete. 

 

After completion, you will be thoroughly evaluated by one of our therapist to determine the cause and solution to your symptoms. You and your therapist will then discuss your plan of care which includes the number of times each week and how long you will need to come to therapy as well as any exercises or modifications to be performed between sessions. Your initial evaluation will take approximately one hour. 

HELPFUL TIPS

  • Arrive 10 minutes early in order to complete required forms prior to seeing the therapist

  • Bring a list of medications if you regularly take multiple medications

  • Wear comfortable clothes and shoes that allow ease of movement

Just download, print, complete, and bring these with you for your first appointment. If you are unable to do so, these forms will be available when you arrive to the clinic.
